Community Staff Nurses - Band 5 – £2000 Golden Hello

Sutton Community Health Services is a Division of The Royal Marsden NHS Foundation Trust. We deliver services to approximately 190,000 residents across the outer London borough of Sutton. Our vision is to promote independence and improve the health outcomes of our patients, to deliver high quality excellent co-ordinated care, across a wide range of health conditions including end of life care, that is patient centered and to provide our services within the heart of the patient’s community.

This Team will transition to be part of Sutton Health and Care Provider Alliance in April 2019. Sutton Health and Care (SHC) is an alliance between local partner health and social care organisations, which will be Hosted by Epsom and St Helier University NHS Trust from April 2019.  The SHC Alliance was formed so that local health and care providers can work together to deliver high quality health and social care for Sutton residents and ensure that care will be coordinated around the needs of the person and will encompass all elements of care whilst supporting people in their homes to be independent and health for as long as possible. From April 2019 this service, will be transferred under a TUPE process to Epsom and St Helier University NHS Trust.  If you have any queries regarding this please contact Chris Dyson
