This is an exciting opportunity to join the adult community dietetics team in Sutton.

We are looking for 0.8WTE (30 hours per week) Band 4 Dietetics Support Worker. You should have a demonstrable interest in nutrition, health and well-being particularly in relation to the older person.

Under the supervision of the Dietetic team you will be:

Undertaking nutritional reviews in the home and care home environment to assess the effectiveness of dietetic care plans.

Providing basic dietary advice.

Weighing patients in the home environment to monitor effectiveness of dietetic care plans. This will require the use of specialist weighing equipment such as hoist and wheelchair scales.

Undertaking basic nutrition screening and information gathering to assist the team with triaging referrals.

Assisting the dietetic team with any ad-hoc duties required to ensure the effective and efficient running of the service

Based at Roundshaw Health Centre, Wallington you should have a full driving license and access to your own car for work use. You should have a flexible approach to meeting the needs of the patients and the dietetic team. You should have experience of working autonomously. You will have a keen interest in nutrition, health and patient care and will be able to demonstrate excellent written and verbal communication skills.

You should have a minimum of an NVQ Level 3 qualification in health and social care. This position is not suitable for a newly qualified/registered Dietitians due to the need to provide consistency over time to our patient population.

We are committed to professional development, offering regular in-service training.  Internal and external courses are supported and there will be opportunities to develop the skills required for this post.

This Team will transition to be part of Sutton Health and Care Provider Alliance in April 2019. Sutton Health and Care (SHC) is an alliance between local partner health and social care organisations, which will be Hosted by Epsom and St Helier University NHS Trust from April 2019.  The SHC Alliance was formed so that local health and care providers can work together to deliver high quality health and social care for Sutton residents and ensure that care will be co-ordinated around the needs of the person and will encompass all elements of care whilst supporting people in their homes to be independent and health for as long as possible. From April 2019 this service, will be transferred under a TUPE process to Epsom and St Helier University NHS Trust.
