Sutton Community Health Services is a Division of The Royal Marsden NHS Foundation Trust. We deliver services to approximately 190,000 residents across the outer London borough of Sutton. Our vision is to promote independence and improve the health outcomes of our patients, to deliver high quality excellent co-ordinated care, across a wide range of health conditions including end of life care, that is patient centered and to provide our services within the heart of the patient’s community. 

We recognise that our nurses play a vital role in our patient pathway and in recognition of this we offer a range of benefits and fantastic learning and development opportunities. The post holder will work as part of our out of hours community integrated service that is responsive to the needs of housebound patients within a defined caseload. They will work to reduce inappropriate hospital admissions, minimise hospital lengths of stay and support vulnerable clients at home by undertaking comprehensive holistic assessment and care plans in order to facilitate and coordinate packages of care in partnership with GP's, patients and health and social care colleagues. The out of hours service operates from 4.30pm to 7.30am over 7 days a week. The successful candidate will work shifts within these hours.
